1. Annual results or management report that includes data on ongoing and completed projects, income, contracts executed, subsidies and contributions received, as well as goals achieved for the benefit of the community.

2. The financial statements of the entity for the year 2023 duly approved by the highest administrative body.

3. Certification of Requirements of the legal representative or tax auditor in which it is shown that they have complied with all the requirements during the respective year, as ordered by numeral 13 of paragraph 2 of article 364-5 ET. It is NOT necessary to attach the income tax return.

4. Copy of the public deed, private document or incorporation document stating that it is legally constituted. Only if it is modified in relation to the one previously submitted.

5. Certificate of existence and legal representation issued by the competent authority, which includes the administrators, representatives and members of the board or governing body where applicable. Applies to Cooperatives that submit their first ESAL update after having made a change to one of the members.

6. Certification by the Legal Representative of the judicial records and declaration of expiration of state contracts of the members of the board of directors, founders, legal representatives or members of the management bodies in accordance with numeral 3 of article 364-3 ET

7. Minutes of the assembly or highest governing body indicating the status of permanent allocations for previous taxable years in accordance with article 1.2.1.5.1.27 of Decree 1625 of 2016. Only applicable when applicable. (Cooperatives should not attach it)

8. Copy of the statutes or minutes of the highest governing body where it is indicated: (minutes)

a) meritorious activity with free access to the community

b) that their contributions are not refundable

c) that their surpluses are not distributed under any modality

d) identification of management positions. Decree 2150 of 2017, Article 1.2.1.5.1.8, numeral 4
